MEDICATION SAFETY INFORMATION & CONSENT

THIS DOCUMENT CONTAINS IMPORTANT SAFETY INFORMATION ABOUT PRESCRIPTION AND OVER-THE-COUNTER PRODUCTS AVAILABLE THROUGH ADEGEN. PLEASE READ IT CAREFULLY BEFORE INITIATING A CONSULTATION OR USING ANY PRODUCT. THIS INFORMATION DOES NOT REPLACE THE INDIVIDUALIZED ADVICE OF YOUR HEALTHCARE PROVIDER OR THE MEDICATION-SPECIFIC SAFETY INFORMATION INCLUDED WITH YOUR PRESCRIPTION.

1. ABOUT COMPOUNDED PRESCRIPTION MEDICATIONS
Prescription hair loss treatments available through Adegen are compounded medications prepared by the applicable licensed pharmacy. The following disclosures apply to all compounded prescription products available through the Adegen platform:
• Compounded medications are not FDA-approved drug products. The FDA does not verify the safety, effectiveness, or quality of compounded medications.
• Compounded medications are not subject to the same safety, effectiveness, and manufacturing review required for FDA-approved drug products.
• Compounded medications are prepared specifically for individual patients based on a valid prescription from a licensed healthcare provider. Each formulation is customized to the prescribing Provider's specifications and may differ in concentration, combination, or delivery method from commercially manufactured medications.
• Compounded medications are only dispensed following a telehealth consultation with and a prescription issued by an independent licensed Provider. The decision to prescribe any medication rests solely with the Provider based on their independent clinical judgment.
• All compounded medications dispensed through the applicable licensed pharmacy are prepared in compliance with applicable state pharmacy laws and applicable USP standards for compounding.
Prescription treatments available through Adegen may include topical formulations, oral medications, or both, as determined by your Provider. Each category of prescription hair loss treatment carries its own potential benefits, risks, and side effects. You are strongly encouraged to discuss all questions about your specific prescribed treatment — including its ingredients, mechanism of action, potential side effects, and alternatives — with your Provider before and during treatment.
2. MEDICATION-SPECIFIC SAFETY INFORMATION
This document provides general safety disclosures applicable to all prescription products available through Adegen. It is not a complete or exhaustive description of the risks, side effects, or contraindications associated with any specific medication or formulation.
Detailed, medication-specific safety information will be provided to you in the following ways:
• In the safety information leaflet included with your prescription shipment, prepared by the applicable licensed pharmacy
• Through your Provider during and following your telehealth consultation
• On the applicable product information pages at adegen.com
You are responsible for reading all medication-specific safety information provided with your prescription before use. If you have questions about the safety information included with your shipment, contact your Provider through the Adegen platform or contact us at support@adegen.com and we will assist in routing your inquiry.
THIS IS NOT A COMPLETE LIST OF SIDE EFFECTS. THERE MAY BE ADDITIONAL SIDE EFFECTS NOT DESCRIBED IN THIS DOCUMENT. YOUR PROVIDER AND THE MEDICATION SAFETY INFORMATION INCLUDED WITH YOUR PRESCRIPTION ARE YOUR PRIMARY SOURCES OF MEDICATION-SPECIFIC SAFETY GUIDANCE.
3. GENERAL SAFETY WARNINGS
The following general safety warnings apply to all prescription hair loss treatments available through Adegen, regardless of the specific medications prescribed to you.
3.1 Pregnancy, Breastfeeding, and Reproductive Health
Several prescription medications available through Adegen are contraindicated during pregnancy, for women who may become pregnant, or during breastfeeding. Some medications in this category carry the risk of serious harm to a developing fetus if handled or used by women of childbearing potential, even through skin contact.
You must disclose your pregnancy status, reproductive intentions, and breastfeeding status to your Provider during your intake questionnaire and at any follow-up consultation. Do not use any prescription product without first discussing these factors with your Provider if any of these conditions apply to you. If you become pregnant during treatment, discontinue use and contact your Provider immediately.
3.2 Drug Interactions
All medications, including topical formulations, have the potential to interact with other medications, supplements, vitamins, and herbal products you may be taking. You must disclose all current medications and supplements to your Provider during your intake questionnaire and at any follow-up consultation. Failure to disclose current medications and supplements may result in an unsafe treatment recommendation.
If you are prescribed a new medication by another healthcare provider while using Adegen treatments, inform that provider of all medications you are currently taking, including any Adegen prescription products.
3.3 Known Allergies and Sensitivities
You must disclose all known drug allergies and sensitivities to your Provider during your intake questionnaire. If you experience signs of an allergic reaction to any Adegen product — including rash, hives, itching, swelling of the face or throat, difficulty breathing, or anaphylaxis — discontinue use immediately and seek emergency medical care. Do not restart the product without consulting your Provider.
3.4 Off-Label Use
Some prescription medications available through Adegen are prescribed for uses that are not specifically approved by the U.S. Food and Drug Administration ("FDA") for that route of administration, formulation, or patient population. This is known as off-label prescribing and is a legal, common, and accepted medical practice. Your Provider will determine whether an off-label use is appropriate for you based on their clinical judgment and the available evidence.
Where a medication is being prescribed for an off-label use, your Provider will discuss this with you during your consultation. You are encouraged to ask your Provider any questions you have about the evidence supporting the prescribed use before agreeing to treatment.
3.5 Cardiovascular and Systemic Considerations
Some hair loss medications may have cardiovascular or systemic effects, particularly at higher doses or in individuals with pre-existing health conditions. You must disclose any history of cardiovascular disease, high or low blood pressure, kidney disease, liver disease, or other significant health conditions to your Provider during your intake questionnaire. Your Provider will assess whether any prescribed medication is appropriate given your health history.
3.6 Initial Shedding
An initial increase in hair shedding is a commonly reported and typically temporary response to starting certain hair loss treatments. This is sometimes referred to as a "dread shed" and generally occurs during the first two to eight weeks of treatment. This shedding does not indicate treatment failure and typically resolves with continued use. If you experience significant or prolonged shedding, contact your Provider.
3.7 Results and Expectations
Hair loss treatment requires consistency and patience. The following expectations apply to all Adegen prescription and over-the-counter treatments:
• Most patients who respond to treatment begin to see measurable results after three to six months of consistent daily use. Earlier results are possible but not guaranteed.
• Results vary significantly between individuals based on genetics, degree of hair loss, age, consistency of use, and other factors.
• No treatment available through Adegen guarantees hair regrowth or prevention of further hair loss for any specific individual.
• Discontinuing treatment will typically result in a gradual return to the pre-treatment rate of hair loss over time.
You should discuss realistic expectations for your specific situation with your Provider during your consultation.
3.8 Scalp and Skin Irritation
Topical hair loss treatments may cause scalp or skin irritation, dryness, itching, or redness at the application site, particularly during the initial weeks of use. If irritation is significant or persistent, contact your Provider. Do not apply any topical treatment to broken, irritated, or sunburned skin without first consulting your Provider.
3.9 Photographs
As part of your intake process, you may be asked to submit photographs of your hair and scalp to assist your Provider in evaluating your condition. You agree that:
• All photographs submitted are of yourself and accurately represent your current hair loss condition
• Photographs are taken in good lighting and clearly show the areas of concern
• You will not submit photographs of another person or digitally altered images
Submitting inaccurate photographs may affect the quality of your Provider's evaluation and treatment recommendation. Submitting photographs of another person constitutes misrepresentation and may result in termination of your account.
3.10 Reporting Adverse Reactions
If you experience any adverse reaction, unexpected side effect, or health concern that you believe may be related to an Adegen product, you should:
• Contact your Provider through the Adegen platform messaging system
• Contact your primary care physician or local urgent care facility for non-emergency concerns
• Call 911 or go to your nearest emergency room for severe or life-threatening reactions
• Contact us at support@adegen.com to report any product quality concerns
You may also voluntarily report adverse reactions directly to the FDA through the MedWatch program at www.fda.gov/safety/medwatch or by calling 1-800-FDA-1088.
4. OVER-THE-COUNTER PRODUCTS
Adegen also offers over-the-counter (OTC) hair care and accessory products that do not require a prescription. OTC products do not carry the same clinical evaluation or prescription safeguards as Rx products.
Please read all product labels and accompanying information carefully before using any OTC product. If you have questions about whether an OTC product is appropriate for you given your medical history or current medications, consult your Provider or a pharmacist before use.
5. YOUR RESPONSIBILITY
By using any prescription or over-the-counter product available through Adegen, you accept personal responsibility for the following:
• Providing complete, accurate, and truthful information in your intake questionnaire and all Provider communications
• Disclosing all current medications, supplements, health conditions, allergies, and reproductive status to your Provider
• Reading all medication-specific safety information provided with your prescription before beginning use
• Using all products strictly as directed by your Provider and not exceeding the prescribed or recommended dose
• Contacting your Provider promptly if you experience any adverse reaction, side effect, or health concern during treatment
• Informing any other healthcare providers you see of all Adegen prescription products you are currently using
• Keeping all prescription products out of reach of children
You acknowledge that the quality of the care you receive through the Adegen platform depends in significant part on the accuracy and completeness of the information you provide. Adegen and your Provider cannot be responsible for outcomes resulting from inaccurate, incomplete, or withheld information.
6. CONSENT
By purchasing a prescription product through Adegen, initiating a telehealth consultation, or proceeding past this page, you acknowledge and confirm that:
• You have read and understood this Medication Safety Information & Consent document in its entirety
• You understand that prescription products available through Adegen are compounded medications that are not FDA-approved drug products, and that the FDA does not verify their safety, effectiveness, or quality
• You understand that this document provides general safety disclosures only and is not a complete description of the risks or side effects of any specific medication
• You understand that detailed, medication-specific safety information will be provided with your prescription shipment and that you are responsible for reading it before use
• You have disclosed or will disclose all current medications, supplements, health conditions, allergies, and reproductive status to your Provider
• You understand that results are not guaranteed and will vary by individual
• You agree to use all products as directed by your Provider
• You agree to report any adverse reactions to your Provider, to Adegen, and to the FDA MedWatch program as appropriate
• You understand that this document does not replace the individualized advice of your Provider
